Progress Physiotherapy collects information needed to assess and provide care, maintain clinical records, arrange appointments and billing, and communicate with other providers or insurers when authorized or required. This may include contact details, relevant health history, assessment findings, treatment goals and payment or insurance information.
Safeguarding and access
Access to records is limited to authorized staff. The clinic uses administrative, physical and electronic safeguards for personal information. You may contact the clinic about access to, or correction of, your record, subject to applicable legal and professional requirements.

The Book buttons take you to Cliniko, the clinic’s external booking service. Review the information shown there before submitting an appointment request. This website does not include a patient intake form. For questions about website data and online booking, contact the clinic.
